Simply print & complete our 'Order Form' or click the 'Add to Cart' button and follow through with our secure (SSL) shopping cart, complete the required fields, print, include the information within your signed PO. Your PO should be accompanied with full payment and with the following information:

2) Order signed & dated by authorizing officer
3) Photo copy of check to be mailed
4) Preferred qty
5) Color
6) Size
7) Ship to address
8) Contact name
9) Primary phone number
10) Email address
11) Need by date
After receiving your information we will fax/email you an invoice indicating your shipping cost and complete total due or you can simply use our shopping cart as a calculator. Tax exempt statuses are also accepted and all forms must be mailed or faxed to us prior to completing your order which may take up to 2 to 8 weeks for orders of 24 to 60 chair pockets. Please contact us for the completion time for 60 or more chair pockets.
